Tadalafil: Function & Applications

1. Mechanism of Action (How It Works)

Tadalafil is a PDE5 (phosphodiesterase type 5) inhibitor, which enhances blood flow by preventing the breakdown of cGMP (cyclic guanosine monophosphate).

  • For Erectile Dysfunction (ED):

    • Sexual stimulation → Nitric oxide (NO) release → cGMP production → smooth muscle relaxation in penile arteries → increased blood flow → erection.

    • PDE5 inhibition sustains cGMP levels, prolonging the erectile response.

  • For Benign Prostatic Hyperplasia (BPH):

    • Relaxes smooth muscles in the prostate and bladder, improving urine flow.

    • Also has mild anti-inflammatory effects on the prostate.

  • For Pulmonary Arterial Hypertension (PAH): (Brand: Adcirca®)

    • Inhibits PDE5 in the lungs → vasodilation → reduced pulmonary artery pressure → improved exercise capacity.

4. Important Considerations

✔ Avoid with nitrates (risk of severe hypotension).
