Glibenclamide, also known as glibenclamide, is a sulfonylurea based hypoglycemic drug. Its main efficacy and effects are as follows:
1. Lowering blood sugar: Glibenclamide stimulates pancreatic beta cells to secrete insulin, increases portal vein insulin levels, or has a direct effect on the liver, inhibiting hepatic glycogen breakdown and gluconeogenesis, reducing hepatic glucose production and output, thereby lowering fasting and postprandial blood sugar. In addition, it may also increase the sensitivity of extrapancreatic tissue to insulin and the utilization of sugar, further improving the hypoglycemic effect.
2. Improving pancreatic islet cell function: Glibenclamide can increase the sensitivity of pancreatic islet cells to glucose, promote their synthesis and release of insulin, and thus improve the function of pancreatic islet cells.
3. Weight loss: Glibenclamide can reduce the accumulation of triglycerides in the liver and promote fat breakdown, thereby helping to control weight. However, it should be noted that studies have also shown that some patients may experience weight gain after long-term treatment.
4. Delay the progression of nephropathy: glibenclamide has antioxidant effect, which can protect the kidney from oxidative stress damage, thus slowing down the development of diabetes nephropathy.
5. Improving blood lipid abnormalities: Glibenclamide can reduce serum total cholesterol and low-density lipoprotein cholesterol levels, while not affecting high-density lipoprotein cholesterol concentration, thus having a positive impact on people with blood lipid abnormalities.

Glibenclamide has some side effects and precautions. Due to its strong hypoglycemic effect, it is most prone to hypoglycemic reactions, so caution should be exercised when using it. Especially for elderly people, patients with renal insufficiency or liver dysfunction, medication dosage should be adjusted under the guidance of a physician, and blood sugar and liver and kidney function should be monitored regularly. In addition, glibenclamide is not applicable to insulin dependent diabetes patients with acute complications, pregnancy, and severe liver and kidney dysfunction.
